Can one avoid the implant of a lens entirely with cataract surgery?

Not if you want to see again. The problem is that your natural lens has fogged over, and it is not something that can be scraped off somehow. The only option is to replace it, and that means a new lens.

Sixty years ago the ruined lens was merely removed, but the result was that the patient was stuck with wearing huge, thick glasses that left their vision severely impaired. Thank goodness for the discovery of artificial lenses!
